About CP Australia

Coordination Practice Australia (CPA) is an independent service provider focused on Level 3 Specialist Support Coordination for people with complex, high-risk, or multi-agency support needs.

We work alongside participants, families, carers, and referring professionals to untangle complex plans, reduce barriers, and build sustainable support systems that last beyond the coordination period. Our approach is trauma-informed, strengths & rights-based, and grounded in practical problem-solving. We are committed to transparency, ethical practice, and clear communication.

What we do

CPA provides targeted Level 3 Specialist Support Coordination for NDIS participants with high and/or complex support needs

For participants facing:

  • Complex health, disability, or psychosocial needs

  • High-risk situations or crisis response

  • Multiple service providers and systems (health, justice, child safety, housing, etc.)

  • Barriers to plan implementation or service access

Our work focuses on:

  • Clarifying goals and plan budgets

  • Mapping and resolving risks and barriers

  • Coordinating across multiple agencies and providers

  • Building participant and carer capability to manage supports independently

  • Supporting plan reviews with evidence and clear recommendations.

We do not:

  • provide direct care or therapy services.

    - This ensures our advice remains independent and focused solely on the participant's interests.

Participants, families, or professionals complete a short enquiry form or call us directly. We confirm service area, eligibility, and current capacity

We arrange a no-obligation conversation to understand needs, risks, and goals, and to explain how specialist support coordination can help.

3. Service agreement and startup

If CPA is a good fit, we issue a service agreement, confirm funding in the NDIS plan, and begin coordination work with a clear plan of action.
